As a fully digital practice, a patient’s images and reports are almost instantly available to the referring clinician.
We now offer our patients access to their images via our imaging portal as well, please complete the below form to request access to your report and images.

Please note that due to Patient Confidentiality, we are only able to release images and reports to the patient directly or the relation with the patient needs to be verified. Please consult with our imaging team for verification requirements.
